Enrollment

The Class B enrollment forms are for enrolling eligible children born after December 18, 1971.

To receive an enrollment form, please call our Shareholder Records office at (907) 243-2944 or (800) 478-2171 (toll-free) or send an email to shareholderrecords@kuskokwim.com. A paper form can also be picked up from either our Anchorage or Aniak office.

Completed forms can be submitted by email, mail, fax or in-person.

Direct Deposit

If you would like to get your TKC distributions deposited directly into a bank account instead of receiving a check in the mail, please complete the Direct Deposit Form and send it to us by email, mail, fax or in-person.

change of address

If you have recently moved to a new address, changed your phone number, or if you have a new email address, make sure your Shareholder record is up-to-date by sending a completed Change of Address Form by email, mail, fax or in-person. You can also update your address and other information online using our Shareholder Portal MyTKC.com.

SMS Two-Factor Authentication Form

This form enrolls your mobile phone number to receive one-time verification codes for account security. These SMS messages are used only for login verification, account verification, and password resets. No marketing, promotional, announcements, or informational messages will be sent using this phone number.

If you have experienced a legal name change, please send us a completed Name Change Form along with a copy of a legal document (marriage certificate, divorce decree or court order) authorizing your name change by email, mail, fax or in-person.

Stop Payment Form

If you have not received your distribution check yet and would like to issue a stop-payment and have the check reissued, complete the Stop Payment Form and return to TKC by email, mail, fax or in-person.

TKC Stock Will Form

A Stock Will allows Shareholders who own Class A shares to pass their shares through inheritance. All Shareholders who hold Class A stock should exercise this right by completing a TKC stock Will. This form does not apply for Class B shareholders. Completed forms can be sent to us by email, mail, fax or in-person.

BEREAVEMENT ASSISTANCE APPLICATION 

TKC’s Bereavement Assistance is offered to family members of a deceased TKC Shareholder, or a TKC Shareholder’s child who is one-year-old or younger, who has passed away to assist with funeral or other related expenses. Applicants can be a spouse, mother, father, sister, brother, son, daughter, or legal guardian of the deceased. The Bereavement Assistance Program will provide a one-time benefit in the amount of $1000 to be used to assist with funeral or other related expenses as the family deems fit. The Bereavement Assistance application requires documentation demonstrating proof of death, which can be a declaration of death certificate, a letter from a hospital, funeral home, clinic, Tribal or community leader, medical examiner, a published obituary, or a police/state trooper report.

Annual Meeting Materials

In order to save paper and reduce significant printing and mailing costs, TKC Shareholders have the option to receive Annual Meeting materials electronically or receive one set of annual meeting materials per Shareholder household. Shareholders can send completed forms to us by email, mail, fax or in-person. Shareholders may revoke consent to receive materials electronically or to receive one set per household at any time.

MyTKC.com is a free, online resource available to all eligible TKC Shareholders and Fiduciaries. If you don’t already have an account, creating one is quick and simple. All you need is your TKC ID number or SSN, your date of birth and an email address. 

From your MyTKC.com account, you can:
